Subject: Data Request No. 1 - Information about California Interconnected VoIP carriers
Pursuant to Public Utilities (Pub. Util.) Code Section 314 (a) and (b), Communications Division
(CD) requests all Interconnected VoIP carriers with informal registration pursuant to Pub. Util. Code
§ 285

to respond to the information requested and instructions identified below.
A. Data Request Information
1. Utility ID and Utility Name of Respondent
2. Type of Interconnected VoIP: Fixed Interconnected VoIP, Nomadic Interconnected VoIP, or
a combination of both fixed and nomadic.
3. Facilities status: full facilities-based, limited-facilities-based, or reseller.
4. Identify if the respondent also has an existing wireline operating authority in California
granted via Certificate of Public Convenience and Necessity (CPCN) application pursuant to
Pub. Util. Code § 1001 or Simplified Registration pursuant to Pub. Util. Code § 1013.
Provide Utility Name(s), Utility ID number(s) and Decision Number(s) which granted the
authority.
5. Are any customers currently being served under the Respondent’s Utility ID? If so, provide
customer active access line count and type that it serves (business and/or residential).
6. Provide the contact’s name, title, email and phone number of the person to be contacted if
there are questions or follow up regarding the responses provided.
B. Definition of Terms
1. Nomadic Interconnected VoIP service refers to the type of non-fixed interconnected VoIP
service, such as Vonage’s DigitalVoice or other IP-enabled services having the same
capabilities, subject to In the Matter of Vonage Holdings Corporation Petition for
Declaratory Ruling Concerning an Order of the Minnesota Public Utilities Commission
(“Vonage Declaratory Ruling”)

and have the following features:

DVSutility type code should provide a response to the data request.

a. Service Location – Service may be used from more than one location or at multiple
locations anywhere.
b. Access – Can be accessed from any broadband connection.
c. Equipment – Service is provided to nomadic (portable) IP compatible communication
devices.
d. Customer/Call Location – Service provider cannot track exact location of calls.
2. Facilities-based carriers are defined as carriers that own or control facilities used to provide
communications for compensation, including the line to the end-user’s location.

Full-
facilities-based carriers install telecommunications facilities that require full CEQA review;
whereas Limited-facilities based carriers install telecommunications facilities in existing
conduit or other structures.

3. Resellers are defined as carriers who do not own transmission facilities but provide service
under their own name using the facilities of another provider. They do not directly own,
control, operate, or manage any conduits, ducts, poles, wires, cables, instruments, and
appliances in connection with or to facilitate communication by telephone.

C. Due Date: On or before 5pm (PST) February 22, 2024
D. Data Request Response format
1. Complete the CPUC DVS Section 285 Carrier Data Request Response” form provided with
this data request and save it as a Microsoft Excel spreadsheet format (.xlsx/.xls). The form is
available for download at: https://www.cpuc.ca.gov/industries-and-topics/internet-and-
phone/voip-providers
2. Submit the completed form via email to cdcompliance@cpuc.ca.gov. The Subject Line for
email: [UXXXXC] [Carrier Name] DVS Section 285 Data Request Response.
E. Submission of Confidential Information
Responses to this data request will be treated as public unless the respondent complies with the
requirements for submission of confidential information pursuant to Decision (D.) 16-08-024, as
modified by D.17-05-035, and General Order 66-D.

All alleged confidential information should
be clearly identified and substantiated with a confidentiality declaration, among other
confidentiality requirements set forth in the aforementioned Commission orders.
