 Only one parent is U.S. citizen and birth was in wedlock: Select Requirement A

 Only one parent is U.S. citizen and birth was out of wedlock: Select Requirement A

 Both parents are U.S. citizens prior to the birth of the child and birth was in wedlock: Select Requirement B

 Both parents are U.S. citizens prior to the birth of the child and birth was out-of-wedlock: Select Requirement B

Please note: Application forms together with copies of documents need to be mailed to the Consular Section. DO NOT send

the originals, bring them to the interview.

 U.S. parent’s naturalization certificate (if applicable)

 Non U.S. parent’s passport or ID

 Applicant’s registered birth certificate

 English translation of applicant’s birth certificate

 Marriage certificate (if birth was in wedlock)

- English translation if other than English language

 Divorce decree (if applicable)

- English translation if other than English language

 Mother’s prenatal records

 U.S. parent’s evidence of physical presence in USA

 Family photos

 Evidence of physical presence in the same location at

the time of conception (if birth was out of wedlock)

 Original bank draft (DS-11 fee)

 DS-11 with photo (2”x2”)
