Introduction
This document provides an overview of the vaccines
required to aend schools in Washington state and the
vaccines that children, from birth to age 17, typically
receive in Ukraine. In addion to comparisons between
the schedules, this document provides examples of what
vaccine records may look like when wrien in Ukrainian
and Russian. When assessing a child’s immunizaon record
to aend school in Washington state, presume that the
child has not received the vaccine in queson, unless the
parent presents a record.
When requesng immunizaon records from parents or
guardians, it is important to note that Ukraine does not
issue a naonal immunizaon card, and vaccine records
vary by clinic and year of vaccine administraon. When
asking families to provide immunizaon records, be ready
to review records on dierent types of forms, either
printed or handwrien. Many records will be provided on
forms created by the hospital or clinic where the vaccine
was received.
You may also be presented with a medical book, which
is a record maintained for children, documenng their
medical history and vaccinaons (many adults connue to
keep these for themselves as well). Vaccine informaon
is typically recorded on the rst few pages of the book
and is usually handwrien. Parents and guardians living
in Ukraine may choose to keep these books or have their
providers oce keep them.
Guidelines for Review
When reviewing vaccine records, keep the following
in mind:
Avoid confusing month and day. Ukraine uses the same
calendar as the United States, but dates are generally
recorded using the European format of DD/MM/YY.
Review both the original vaccine record and the
translaon (if available), to ensure general consistency;
for example, name, date of birth, and total number of
vaccines.
If a translaon is not available, reference the Centers
for Disease Control and Prevenon’s (CDC) Pink Book
for generally used terms for vaccines in Ukrainian or use
a reputable translaon service. See Pink Book Appendix
B: Vaccines.
If a vaccine record is not available, reference this CDC
guidance: Approaches to evaluaon and vaccinaon of
persons vaccinated outside the United States who have
no (or quesonable) vaccinaon records.*
More informaon about childhood vaccinaon in Ukraine
can be found on the Ministry of Health Ukraine and WHO
websites. Please note that older children and adults may
have received dierent vaccines. For informaon about

Washington State Condional Status Catch-Up
Schedule for Age 4 Years and Older
Children can enter and stay in school or child care in condional status if they are catching up on the vaccines required
to document immunity. Condional status allows a child to aend school or child care while waing for their next
vaccine dose according to the naonal immunizaon catch-up schedule. When the child’s next vaccine dose comes due,
they have 30 calendar days to turn in documentaon showing their vaccinaon. If a child is catching up on mulple
vaccines, condional status connues in a similar manner unl all of the vaccines needed to document full immunity are
completed.
Before aending in condional status, a child must have at least the rst dose of the vaccine series. Addionally, the
child can’t be currently due for any addional doses. Condional aendance is conngent upon documentaon of the
remaining required immunizaons as shown in the melines below.

1. Valid documentaon includes medical records showing vaccinaon, evidence of immunity to the disease the vaccine prevents, or a completed Cercate of Exempon (COE) form.
2. Three doses of polio vaccine complete the series if dose 3 was given on or aer the 4th birthday AND at least 6 months aer dose 2.
3. DTaP is used through 6 years of age. At age 7 and older Tdap or Td is used, at least one Tdap must be included.
4. Four doses of DTaP vaccine complete the series if dose 4 was given on or aer the 4th birthday AND at least 6 months aer dose 3.
5. At age 7 and older the series can be completed with three doses IF dose 1 was given on or aer 12 months of age AND dose 3 is at least 6 months aer dose 2. One of the three doses
must be a Tdap.

Ukrainian Vaccination Record Examples
As noted in the introducon to this document, Ukraine does not maintain standardized immunizaon records, or issue
a naonal immunizaon card. Records vary by locaon, administraon meframe, and provider. You may be presented
with immunizaon records in dierent formats, which include vaccines recorded in a medical book (gure 1) or in
an immunizaon form (gure 2). In the following examples from dierent providers, an individual’s records may be
handwrien and include lifelong medical records.

COVID-19
COVID-19 vaccinaon is required for people from Ukraine ages 6 months and older to enter the United States. In some
cases, vaccine informaon may be a handwrien record, or the individual may have received a printed COVID-19
cercate. See gures 4 and 5 below for examples.

Diphtheria, Tetanus and Pertussis
The roune schedule for pertussis vaccinaon in Ukraine includes four doses of the DTaP vaccine administered between
2 and 18 months. This is followed by one dose of DT vaccine at 6 and one dose of TD at age 16.
Children arriving in the United States who were vaccinated on this schedule will need an addional DTaP (ages 4 - 6) or
Tdap (age 7+) if they have received pertussis vaccine at age 4 or aer. Children who receive a Tdap at ages 7 - 9 sll need
another Tdap at age 11. See the Washington State Department of Health Individual Vaccine Requirements Summary for
more informaon.

Hepatitis A and B
Similar to Washington state requirements for aending school and child care, hepas A vaccinaon is not required in
Ukraine, so it is rare to nd records containing an example of hepas A vaccinaon. Hepas B vaccinaon is required
in Ukraine, although not many people receive it. Ukraine’s schedule is consistent with the U.S. schedule with regard to
hepas B vaccinaon. Note that you may see hepas B vaccine wrien in dierent ways, including in English.

HPV
Similar to Washington state requirements for aending school and child care, HPV vaccinaon is not required in Ukraine,
but individuals can choose to be vaccinated.
Inuenza
Like Washington state requirements for aending school and child care, inuenza vaccinaon is not required in Ukraine,
but individuals can choose to be vaccinated.
Meningococcal
Meningococcal ACWY vaccine is not required in Ukraine or for aending school in Washington state, but individuals can
choose to be vaccinated. Please see the vaccinaon schedules for dierences in administraon.
Polio
Ukraine’s immunizaon schedule for poliomyelis requires inacvated polio vaccine (IPV) for the rst two doses, with
one dose at 2 months and a second at 4 months, followed by either IPV or oral polio vaccine (OPV) for the remaining four
doses. (See the Ukraine vaccine schedule in this document for addional guidance.) The U.S. schedule recommends that
children receive four doses by the age of 6 years, and it is a requirement for entering child care and school in Washington
state.
Polio Vaccines Given on or After April 1, 2016
Note that any OPV given on or aer April 1, 2016, is not accepted in the United States. To determine which OPV vaccines
may be accepted and determine addional doses needed, see Guidance for Assessment of Poliovirus Vaccinaon
Status and Vaccinaon of Children Who Have Received Poliovirus Vaccine Outside the United States. Children may need
addional doses of IPV to complete the polio series to meet requirements for aendance in Washington state schools.

Bacillus Calmette–Guérin (BCG)
BCG vaccine is given as a single dose at 3-5 days old in Ukraine. This vaccine is not required for aendance in Washington
state schools, so these notes can be disregarded when found within an immunizaon record. It is included here for
reference only.

Varicella (Chickenpox)
Ukraine’s immunizaon schedule does not include varicella vaccinaon. Children who have not received vaccinaon for
chickenpox need to receive this vaccine to meet Washington state school and child care requirements.
Pneumococcal Conjugate Vaccine (PCV)
Ukraine’s immunizaon schedule does not include PCV. Children younger than 5 years who have not received their PCV
vaccine series need to receive this series to meet Washington state school and child care requirements.
