Recommendation for Undergraduate Admission Form

Guidelines:

Many students send letters of recommendation as a part of their application for admission. Most campuses do not

 requestlettersofrecommendationfromapplicantsbutmayaskfortheminspecicsituations.

2. The Recommendation for Undergraduate Admission form is accepted by most campuses in the UW System.

UW-Madison provides their applicants with an online Letter of Recommendation Form at

https://www.admissions.wisc.edu/apply/recommendations/submit/index.php which should be used by all applicants

to UW-Madison.

3. Individual letters of recommendation written by teachers, counselors, and other educators are also accepted at

most UW campuses.

4. If possible, please submit the form electronically as an attachment when submitting the applicants’ electronic transcript.

5. If not included as an attachment when sending the electronic transcript, the Recommendation for Undergraduate

Form should be emailed or sent via U.S. mail to the campus/es to which the student is applying.
