EMSP LICENSE RENEWAL REQUIREMENTS
The information listed below is to assist you in providing the necessary documents along with your
application & appropriate fee to renew your EMS license. It is NOT necessary to provide a criminal
history release form when renewing your license, we already have one on file.
FEE OPTIONS:
Standard two year renewal - $10.00
Please send check or money order made out to ADPH Office of EMS.
One year renewal - $5.00
Please send check or money order made out to ADPH Office of EMS.
All active EMSP must have a form completed by their service’s off-line Medical Director, or an on-line
Medical Director or authorized designee attesting to their mastery of the skills and knowledge of the
Alabama Treatment Protocols. As a part of completing the form, the physician or authorized designee will
randomly select three protocol scenarios from a selection of scenarios provided by the Office of EMS.
The EMSP must satisfactorily verbally complete the selected scenarios based on the Alabama EMS
Patient Care Protocols and the physician or authorized designee must attest to this by signing off on a
protocol certificate including the EMSP’s name, license number, level, and physician’s signature and/or
authorized designee.

Reciprocity Information
EMSP
You must be a Nationally Registered EMSP to apply for the issuance of an Alabama EMS license. The

or faxed to you upon request.
Out-of-State Intermediates are eligible at the EMT level.
Alabama Protocol education is required for ALL levels.
**NOTE** A LICENSE WITH DRIVER ON IT WILL NO LONGER BE ISSUED. DRIVERS
MUST MAINTAIN CURRENT EVOC CERTIFICATION ON FILE WITH YOUR SERVICE.
***On-line renewal will be available from January 1, 2013 through March 31, 2013.
PROTOCOL VERIFICATION
Protocol verification is a combination of physical skills and scenario competency testing. Listed below
are the specific skills that each level should be evaluated on by a Medical Director or Training Officer,
during their licensure period to determine proficiency. New protocol scenarios will be posted on the EMS
website this month.
